MCHD Flu Clinic

The MCHD Mobile Health Clinic will be on-site on Friday, October 23, from 9:00–11:00 a.m.

Please note: This clinic will be for FLU SHOTS ONLY.

If a legal guardian would like their child to receive an influenza vaccine while they are not present, they must complete the attached Minor Consent Form. The form must specifically identify the individual who will be accompanying the child to receive the vaccine (for example, Erica Elam – Oktaha School Nurse).

A copy of the completed consent form will need to be provided for upload to the child’s medical record.

Also attached is a Vaccination Screening Checklist, which must be completed by the child’s legal guardian prior to vaccination.
